I am voluntarily providing my testimonial, and I authorize Jane Yu, L.Ac. to copy, exhibit, publish or distribute my testimonial for purposes of marketing, publicizing clinic services, or for any other lawful purpose. My testimonial may be used in printed publications, multimedia presentations, on websites or in any other distribution media. I hereby waive the right to inspect or approve the finished product, including any written copy, wherein my testimonial appears. I am receiving no financial compensation for providing my testimonial. Jane Yu places no conditions on this authorization. If at any time I would like to revoke my testimonial, I understand I can do so by contacting Jane Yu.
